What does successful community integration of persons with serious and persistent mental illness (SPMI) look like? 

Population: A large percentage present within the chronically homeless and corrections. 

Resources: What is needed to develop, own, and manage supportive housing for this vulnerable population? 

Proof of Concept: What do we need to know that we do not know in order to pilot this successfully for the individual and the community? 

Success: What are affordable housing providers doing now to be successful? Next Steps: What do you need going forward to be successful in sustaining health and housing stability for this population.
