Home Forward is seeking a Case Manager. The Case Manager is a brand new position supports Home Forward’s strategic and operational objectives in the following ways: Offer housing stabilization services to Home Forward’s most vulnerable residents who are at risk of becoming homeless. Provide crisis response, risk assessment and crisis stabilization planning for participants and residents residing in Home Forward housing. Services are provided to individuals experiencing a behavioral health and/or drug and alcohol crisis. This work engages individuals with a focus on supporting trauma informed care, and racial justice.

Some essential job functions include:
- Eviction prevention, including supporting households out of compliance with leases due to mental, emotional and/or cognitive health conditions.
- Conduct assessments and participant-driven planning to meet tenant needs, ensure long-term housing stability, and reach goals.
- Identify and assess risk factors for harm to self or others in the context of the participant/resident’s current psycho-social state via interview with individual and with consideration given to collateral information from resident services, property management and community agencies.
- Develop crisis intervention plans in partnership resident services and property management to bridge the gap in community resources and supports.
- Actively participate in joint case planning and coordination with resident services and property management for residents demonstrating highly disruptive behaviors in the apartment community.
- Initiate engagement and coordinate support and resources for a higher level of care for individuals if deemed necessary.
- Perform related duties and responsibilities as required.

Compensation starts at $25.49 per hour and Home Forward provides wonderful and competitive benefits, including (please see the benefits tab for full details):

Education & Experience Guidelines
Any combination of experience and education/training that would likely provide the required knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be:
- One (1) or more years of experience behavioral health or chemical dependency case management.
AND
- Bachelor’s degree with a focus in behavioral health, or another related field is required.

Environment:
Duties are performed in a public housing site and involve day-to-day interactions with public housing clients in an office and apartment complex. Some remote work may be available. Travel between sites may be required.
Physical:
Incumbents require sufficient mobility to work in a public housing office setting and operate office equipment; sit for extended periods of time; perform repetitive motion of hands and wrists. Must also have sufficient mobility to complete inspections.
Vision:
Vision sufficient to read small print, computer screens and other printed documents.
Supervision Received and Exercised:
Receives General Supervision direction from the Program Supervisor. Does not provide direct supervision.
