What You’ll Do:
We are looking for a Case Manager to assist residents with their successful progress through and graduation from the CPR shelter program. This is an integral position and part of a navigation team. You will be responsible for:
- Performing assessment and intake of all individuals/families applying for shelter.
- Meeting with program participants regularly to help them set goals and develop a case plan then follows-up and monitors their progress.
- Assisting participants to meet their housing goals by helping them to resolve problems and remove barriers.
- Using knowledge of community agencies and services to connect participants with necessary resources.
- Ensuring participants comply with program rules and expectations.
- Working collaboratively with other team members to avoid issues and promote harmony with residents.
- Maintaining mandated data collection and reporting as well as records regarding services provided. Issues exit notices and approves extensions consistent with policies.
- Providing de-escalation and crisis intervention services as needed.
- Oversee and delegate to volunteers or other employees providing assistance to the Case Manager.
What You’ll Bring:
- HS education or equivalent required.
- BA or MA in counseling, social work or related field of study preferred.
- CCM preferred.
- At least one year of successful case management experience, work with low-income or homeless population preferred.
- Demonstrated ability to assess people in crisis and work productively with them to establish goals and develop plans.
- Outstanding interpersonal skills and ability to effectively manage conflict. Must be able to maintain a calm, empathetic and respectful demeanor.
- De-escalation and crisis management skills required.
- Excellent written and verbal communication skills. Ability to effectively communicate with a diverse population.
- Strong attention to detail, prioritization and organizational skills.
- Outstanding analytical, critical thinking and problem solving skills.
- Must be adaptable and flexible.
- Bilingual (English/Spanish) preferred.
- Ability to travel to other worksites or meeting locations.
