The Vancouver Housing Authority (VHA) works closely with local governments and other community partners throughout Clark County to address issues of affordable housing and homelessness and help families break the cycle of poverty.
Who we are looking for
The VHA is seeking a Community Wellness Specialist to work with individuals and families to provide information, health education, and navigational support in accessing community services and other resources in order to adopt healthy behaviors for improving their self-sufficiency. Community Wellness Specialist will work with clients at great risk of having adverse health or housing outcomes, conduct a standardized risk assessment to develop an individual plan; connect client to appropriate care, remove barriers to obtaining care, and measuring the results.
What you’ll do
Community Wellness Specialists (CWS) will maintain a caseload of clients to support their housing goals and overall wellness. CWS’s will utilize motivational interviewing to provide an intake and risk assessment to clients to create an individualized plan to support client’s goals. Services provided include, but are not limited to, one-on-one education with client for resource gathering and improving supports, resource referrals, assisting with and arranging transportation for attending appointments. Appointments can occur within the community, in the client’s home and over the phone.
CWS’s are expected to work collaboratively with other Community Wellness Specialists and Supervisor to make sure that all client needs are identified and addressed. When necessary, CWS’s educate coworkers on issues related to prior experience and specialties to help them better work with and assist clients. CWS’s will attend all trainings and meetings provided by the SWACH Pathways Community HUB. CWS’s are expected to document all client assessments and interactions in a timely manner while maintaining client confidentiality.
All VHA employees are held to a high standard of professionalism. During employment, CWS’s are expected to consistently comply with position and department safety and attendance standards. You will also be expected to establish and maintain productive relationships with employees, clients, vendors and other work related contacts.
What we offer
- Starting pay rate is $3,356.00 to $3,600.00 per month based on experience.
- Full-time, non-exempt, union-represented (OPEIU) position working 40 hours per week, Monday – Friday.

What you’ll bring
- High School diploma or equivalent.
- Minimum of two years’ experience in health education or chronic disease management.
- Experience working with low income, multi-ethnic populations preferred.
- Bilingual preferred.
- Knowledge of community resources in Clark County a plus.
- Valid driver’s license.
