The Resident and Community Services Coordinator – Dahlke Manor position supports Home Forward’s strategic and operational objectives in the following ways:

Plan, coordinate and provide resident, family, community service, community building, and social and educational services to improve the stability, quality of community life, and wellness of individuals and families. Duties are site or program based, vary with site or program needs, and may include, for example,  to assess the needs of individuals and families in order to provide referrals to community service providers and partners; to provide ongoing case coordination and resident communications; to collaborate with site management staff, landlords and other key housing staff to ensure all services are coordinated effectively and consistently; to respond to and intervene in resident problems and crises, to create, develop, and implement community, organization, and program initiatives; and to create and implement resident initiatives designed to build leadership and promote inclusion in the development of communities.

In this context, the incumbent in this position will collaborate with others at all levels of the organization and with external stakeholders in the consideration of significant goals, concepts, initiatives, and other activities that profoundly affect Home Forward, its employees, and the people that it serves.

This position will cover the following properties:

  • Dahlke Manor – Dahlke Manor is a nine-story public housing apartment community with 115 one bedroom homes. This community is reserved for seniors and persons with disabilities.  Located in inner Northeast Portland, it is close to mass transit and Lloyd Center and the shops on NE Broadway.
  • Grace Peck Terrace – Grace Peck is a six-story apartment community with site-based Section 8 subsidy that serves persons 62 and over and persons with disabilities.  It offers 95 one-bedroom apartment homes conveniently located in the Irvington neighborhood of Portland on NE 14th and Hancock St., just minutes from Lloyd Center and other retail services. 
  • Unthank Plaza – Unthank Plaza is a site-based Section 8 apartment community for residents 62 and older and residents with disabilities. The four-story community consists of 80 one-bedroom apartments. Located on the corner of North Williams Avenue, it is minutes from the Lloyd Center retail district and near Emanuel Hospital.

Some of the Essential Job Functions include:
The following tasks are typical for positions in this classification. Any single position may not perform all of these tasks and/or may perform similar related tasks not listed here:

  1. Plan, coordinate and provide an array of social, educational, and community services for residents to improve the stability, quality of community life, and wellness of individuals and families in low-income housing; to oversee and implement the activities of community and/or client-based Home Forward programs.
  2. Complete assessments of residents and link with service providers for further assessment and long term service delivery.
  3. Provide referrals to area service providers and partners, with an emphasis on optimal use of community resources and resident independence in identifying and building support networks beyond Home Forward staff.  When needed, serve as liaison between residents and area service providers/partners to monitor appropriateness of service delivery.
  4. Provide ongoing case coordination and communications or informal arrangements for residents to improve their housing stability, health, safety and progress towards self-sufficiency; determine additional needs or changes in service or care requirements; respond to questions and inquiries from residents and assist when crises occur.

Education & Experience Guidelines

EDUCATION AND EXPERIENCE GUIDELINES:

Any combination of experience and training that would likely provide the required knowledge and abilities is qualifying.  A typical way to obtain the knowledge and abilities would be:

Education:

Equivalent to bachelors OR associate OR post high school trade/specialty studies.  Alternative education and experience qualifications welcome.

Experience:

Three years of increasingly responsible experience in the social work or social services with an emphasis in direct client work with individuals and/or families, case coordination, and program development. Alternative education and experience qualifications welcome.

License or Certificate:

Possession of, or ability to obtain, a valid driver’s license.

Environment
Duties are performed in a public housing site or affordable housing property and involve day-to-day interactions with distressed, disabled, and abusive clients.

Vision
Vision sufficient to read small print, computer screens and other printed documents.
